Release of Liability(Required)

I release the Lexington Blueberry Festival, its organizers, volunteers, sponsors, and the City of Lexington from any and all claims arising from my participation.

Photo Release(Required)

I grant permission for photos and videos taken during the festival to be used for promotional purposes.

Rules Agreement(Required)

I agree to follow all festival rules and regulations and understand that violations may result in removal from the event.

Required for All Participants

A signed liability waiver is required for all festival participants, including vendors, parade entries, pageant contestants, car show entrants, and volunteers. Please complete and submit this form as part of your application. Your participation will not be confirmed until a signed waiver is on file.

Miss Blueberry Pageant contestants (parent/guardian signature for minors)

Any individual or group performing or demonstrating on festival grounds
